File a claim

Reporting a Claim

In order to process your claim, the claim form must completed and supported with the following documents.
  1. Photocopy of shippers original invoice from vendor/supplier
  2. Serial numbers of each item (if applicable)
  3. Photocopy of original retail invoice
  4. Repair invoice or signed certified statement of non-repair from an authorized technician (if applicable)

Please return this information and the completed claim form as soon as possible to the following address.
